Submit your t-shirt designs

T-Shirt Design Guidelines: 

• Entry Deadline: September 30th, 2024
• Send designs to: 
  • Upload to jotform (max. 15MB)
  • Email to publicHealth@co.crawford.wi.gov 
  • Mail to 225 N. Beaumont Rd. Suite 306 Prairie du Chien, WI 53821
• Design print will be limited to 1-2 colors.
• Printed colors to choose from: Purple, Teal, Green, White, Grey, or Black.
  • Purple and Teal are the colors for suicide prevention
  • Green is the color for Mental Health Awareness
• Designs should include: 
  • At leaset one of the following words: 
    • Suicide Prevention/Prevent Suicide
    • Mental Health Matters
    • You Matter
  • Must include CALL, TEXT, CHAT #988
  • Encouraging messages, quotes, phrase, image, etc.
  • NOT to be, "In Memory of", any specific person
  • Enter as many different designs as you wish!

The winning design will go on the front of a t-shirt that will be sold throughout Crawford County, WI and will be shared on the Crawford County Health & Human Services Facebook page. The winner will receive a free t-shirt!

The winner will be announced by the end of October and t-shirts will be available to order in November.
