The Living Well with Chronic Conditions program, offered by the Health Department, is designed to help individuals effectively manage long-term conditions like diabetes, heart disease, arthritis, chronic pain, and depression. Through education and hands-on strategies, we help people improve their quality of life through self-care, coping strategies, exercise routines, and effective communication with healthcare providers.
The Living Well with Chronic Conditions program is focused on providing people with the knowledge and tools they need to take control of their health, manage their symptoms, and live healthier, happier lives. Whether it’s managing daily tasks, dealing with pain, or improving mental well-being, participants will learn key skills to enhance their health and well-being, empowering them to take charge of their health and lead a fulfilling life despite their condition.
SELF-CARE
